The Health Records Department has an opening for a Full-Time Health Records Technician position. The successful candidate must be able to meet the normal requirements of the job and possess the following qualifications:

QUALIFICATIONS NECESSARY:

  • Grade 12 or equivalent acceptable to the Corporation;
  • Registered Certified Health Information Management professional;
  • Current membership with the Canadian Health Information Management Association (CHIMA);
  • Entry level technician willing to expand job knowledge and skills, experience is an asset.
  • Understands and maintains confidentiality;
  • Excellent organizational skills;
  • Proficient use of computer software, including Word and Excel;
  • Effective communications skills, both verbal and written;
  • Ability to perform varied tasks with minimal supervision;
  • Dependable with an excellent attendance record;
  • Physically capable of performing the essential duties of the job.

BASIC RESPONSIBILITIES:

Under the direction of the Supervisor Health Records & Registration is responsible for coding, abstracting and submission of Health Records information using ICD-10-CA/CCI Coding Classification System/Coding Standards.

Under the guidance of the Supervisor of Health Records and Registration is responsible for Medical/Legal Release of Information.

GENERAL DUTIES:

1. Attend and participate in in-service educational sessions and on-the-job training programs, workshops, seminars, as approved.

2. Promote goals and objectives of the corporation.

3. Adhere to the Occupational Health & Safety programs, standards and practices.

4. Adhere to policies and procedures.

5. Adhere strictly to principles of confidentiality.

6. Promote continuous quality improvement principles and participate in ongoing activities.

7. Other duties as directed by the Supervisor Health Records & Registration/designate.

BASIC RESPONSIBILITIES:

1. Assist in preparation, downloading and distribution of statistical data.

SPECIFIC DUTIES:

ICD-10-CA/CCI CLASSIFICATIONS

  • Code and abstract Health Records using ICD-10-CA/CCI Coding Classifications Systems and standards for Discharge Abstract Database (DAD) and National Ambulatory Care Reporting System (NACRS).
  • Perform data quality checks on DAD and NACRS data prior to submission to Canadian Institute for Health Information (CIHI).
  • Perform month end balance of DAD and NACRS data prior to submission to Canadian Institute for Health Information (CIHI).
  • Submit data via the CIHI Web site, relating to DAD and NACRS meeting CIHI and Ministry of Health and Long-Term Care submission schedule.
  • Receive error messages from CIHI, correct and resubmit corrections to CIHI.

Release of Legal/Medical Information

· Receipting and Logging of all Medical/Legal Information Requests.

· Responding to and preparing of all Medical/Legal Information Requests.

· Maintaining all files related to release of information.

· Preparing and submitting invoice for Business Office.

· Obtain proper patient consent.

Statistical

· Prepare monthly Release of Information report.

· Assist Supervisor of Health Records & Patient Information Services in the preparation and distribution of statistical reporting.

Quantitative and Qualitative Analysis of Patient Records

  • Perform Quantitative Analysis on all in-patient records to identify deficiencies.
  • Prepare and distribute deficiency slips for Nursing Deficiencies.
  • Enter all Physician Deficiencies in the Physician Deficiency Log and place charts on Incomplete Record Shelf and/or in Physicians Deficiency Folder accordingly.
  • Perform recheck on patient records once physician has completed, process chart accordingly and make adjustments to the Physician’s Deficiency Log.
  • Prepare and distribute Physician Deficiency Letters on a bi-weekly basis.

REPORTING:

· Assist Supervisor Health Records & Registration in various chart audits

POSITION AVILABLE:

· This Full-Time position will commence: As soon as possible.
