POSITION (Title):

Full Time Quality Safety Risk Coordinator

DEPARTMENT/SERVICE AREA:

Administration – Risk Management

SITE:

Primarily at LaVererndrye General Hospital with

Corporate support for all sites. Travel to Rainycrest,

Rainy River and Emo Health Centres as required

REPORTS TO (Title):

Director, Risk Management

REQUIRED QUALIFICATIONS:

 Registered Nurse with current Certificate of Competence from the College of Nurses of

Ontario.

 Baccalaureate Degree in Nursing.

 Minimum of 3-5 years hands on clinical experience with ability to understand and

navigate the patient care process.

 Patient Safety / Risk Certification preferred. Incumbent required to enroll within 6

months of hire.

 Demonstrated knowledge in the following areas quality/process improvement, risk

management, patient safety, just culture and project management preferred.

 Well developed critical thinking and decision-making skills carrying out activities in an

ethical manner per RHC Ethical Framework.

 Strong interpersonal and communication skills; required to effectively communicate and

collaborate with management, committees, unions, workers, and outside agencies.

Recognized for ability to demonstrate extreme discretion and diplomacy when handling

confidential matters and materials.

 Strong relationship building, facilitation, leadership and negotiation skills.

 Ability to review and analyze data and ability to generate metrics as well as written and

verbal reports.

 Demonstrated self starter with the ability to work in a complex fast paced and variable

work environment, requiring flexibility and potential extended and/or varied work

hours.

 Exceptional organizational and time management skills with the proven ability to

prioritize and manage competing demands and manage and accurately organize time

sensitive documentation.

 High degree of knowledge of relevant legislation, regulations, and jurisprudence in

information management, health and safety and employment, and long-term care,

including, but not limited to the Excellent Care for All Act (ECFAA), Quality of Care

Protection Act (QCIPA), Personal Health Information Protection Act (PHIPA),

Occupational Health and Safety Act (OSHA), Ontario Human Rights Code, Health Care

Consent Act, Workplace Safety Insurance Act (WSIA), and Accessibility for Ontarians

with disabilities Act (AODA), and Fixing Long Term Care Act.

 Advance computer skills proficient using Microsoft Word, Excel, Power Point, Outlook

and Presentation applications.

 Proficiency using risk management software, and human resources information systems

an asset.

 Demonstrated satisfactory work performance and attendance history

BASIC RESPONSIBILITIES:

The QSR Co-ordinator will assist in managing a comprehensive integrated risk management

program through promoting a culture of safety throughout the organization (clinical and nonclinical). The QSR Co-ordinator independently supports the identification, assessment,

mitigation, and monitoring of clinical, operational, financial, legal and reputational risks across

the corporation. This will involve gathering and analyzing patient safety and risk data for

patterns and trends in delivery of care and services, organizational day to day operations to

identify gaps, risks, and areas for improvement. Working collaboratively with leadership, staff,

legal counsel, and external partners the Co-ordinator contributes to organizational resilience,

regulatory compliance and the delivery of safe, high quality patient care while upholding

privacy and confidentiality obligations under applicable legislation.

SPECIFIC DUTIES:

Incident Reporting System:

 Assists in maintaining and monitoring the safety event reporting system on a daily basis.

 Reviews incoming safety reports for completion and accuracy and flags issues for followup wit the most responsible person

 Analyze patient safety data to identify trends and actionable items to advance a just

culture

 Assists in the preparation of reports in the form of tracking, trending, graphs or analyses

regarding patient safety events.

 Reviews all incidents and complaints representing risk exposure to Riverside Health

Care.

 Ensures timely reporting of required events to Health Canada in support of Vanessa’s

Law

Quality, Safety and Risk Management:

 Assists with maintenance and updates to the risk register, risk scoring, monitoring tools

and reports.

 Provides support on an organizational basis through consultation and education on risk

reduction strategies.

 Collaborate with Professional Practice Specialist, Privacy Officer, Infection Control,

Occupational Health & Safety, and other stakeholders to align risk practices.

 Utilizes several quality and risk tools, lean methodologies to ensure quality

improvement and risk management best practices are being followed at all times

 Support interdisciplinary quality of care reviews and incident analysis

 Assists in the timely investigation of select safety events through the implementation of

cause analysis and use of other investigative tools.

 Assist with coordination to support claims investigation, including record review,

witness interviews, documentation, and preparation of factual summaries.

 Provide guidance and support to leaders and staff involved in claims regarding process

expectations, documentation, and timelines

 Maintain accurate records and recommendations and actions required because of an

incident analysis.

 Establishes and maintains effective relationships with staff, managers, and physicians in

order to influence and advance safety practices and behaviours.

 Attends working groups/committees throughout the organization to provide a safety

and risk perspective

 Monitor changes in relevant legislation and standards and access potential

organizational impacts

 Reviews coroner’s inquest reports with the appropriate areas of the organization to

assess the applicability of the recommendations to Riverside Health Care.

 Serves as a resource to managers on effective corrective action design to minimize

event recurrence.

 Performs other position related duties as assigned and/or required.
