REPORTS TO (Title):

Director, Community Support Services

The Community Support Services department has an opening for a Part-Time Medically Stable Transport Driver. The successful candidate must be able to meet the normal requirements of the job and possess the following:

QUALIFICATIONS NECESSARY:

  • A grade 12 Diploma and/or related education or experience acceptable to the Employer.
  • Valid Driver’s License, Class G and a safe driving record.
  • Current certification in the Transportation of Dangerous Goods Course
  • Valid CPR certification.
  • Working Knowledge of Computers.
  • Experience in transporting seniors and/or the physically disabled in a community setting considered an asset.
  • Must be able to work in a Health Care environment.
  • Understand and maintain confidentiality.
  • Demonstrated strong written and verbal communication skills.
  • Demonstrated strong interpersonal skill with the ability to function effectively independently, within a team and with management.
  • Must be able to function effectively in a fast-paced healthcare environment with excellent organizational and time management skills with the ability to multitask and prioritize workload.
  • Dependable with a verifiable acceptable attendance record.
  • Must be willing to work a variety of shifts including evenings, weekends, and stat holidays.
  • Periodically available for short notice call-ins.
  • Knowledge of the resources for seniors available in the community
  • Experience working independently and with minimal supervision.
  • A clear Criminal Record and Vulnerable Sector Screening check
  • Demonstrated to be physically capable of performing the essential duties of the job which include lifting and manoeuvring patient conveyance devices.

BASIC RESPONSIBILITIES:

  •      Primarily and under the direction of the Community Support Services Manager, provide for the safe transportation of medically stable users across the Rainy River District and beyond who require transportation for the purposes of health and social wellness related travel.
  •     Secondary to the delivery of transportation services and under the direction of the Community Support Services Manager or Designate, provide individualized support services and assistance to meet the needs of CSS clients to enable them to remain comfortably in their own home for as long as possible.

SPECIFIC DUTIES:

1. Responsible for the safe transportation of medically stable people to and from appointments as required, to promote health and social wellness.

2. Responsible for maintaining a clean vehicle for operation.

3. Required to complete daily safety check of vehicle and maintain daily safety and mileage logs.

4. Report all vehicle discrepancies to Manager or designate.

5. Follow all rules and regulations pertaining to drivers of transportation services.

6. Deals tactfully and courteously with passengers, staff, and the general public.

7. Collects revenue, completes time and revenue records on a bi-weekly basis.

8. Maintain required documentation.

9. Communicate all relevant information regarding clients, their scheduled appointments/programs to appropriate staff members.

10. Attends staff development programs and meetings as authorized by the Community Support Services Manager.

11. Responsible for following all organizational policies and procedures in performing client care.

12. Participates in the Quality Improvement Program.

13. Carries out work assignments safely and is knowledgeable of and in compliance with relevant regulations, policies, and procedures (e.g., Occupational Health and Safety Act.)

14. Assist clients with personal activities of daily living as defined in the client’s individualized plan.

15. Accepts client assignments from Community Support Services Manager.

16. Performs other duties as assigned.
