Full-Time Interim Regional Transportation Coordinator/Attendant

Director of Community Services

The Community Services/Transportation department has an opening for a Full-Time Interim Regional Transportation Coordinator/Attendant. The successful candidate must be able to meet the normal requirements of the job and possess the following qualifications:

QUALIFICATIONS NECESSARY:

  • Graduate of a health-related degree/diploma/certificate program or equivalent experience and training as determined by Riverside Health Care
  • Knowledge or experience of the regional health care system and community-based resources, experience working with the senior population in the community is considered an asset.
  • Demonstrates strong clinical practice skills and judgement.
  • Demonstrates critical thinking with an ability to assess and respond to client needs and set priorities for care.
  • Excellent organizational and time management skills.
  • Exhibits the ability and competence to work will all computer systems and applications with knowledge and/or experience with electronic medical records documentation.
  • Valid CPR and First Aid certification.
  • Valid Driver’s license, Class G and a safe driving record with a willingness to upgrade to a Class F Driver’s License.
  • Demonstrates ability to maintain strict confidentiality.
  • Dependable with a verified and acceptable attendance record.
  • Demonstrates ability to function effectively independently, within a team, with front line personnel, and management.

BASIC RESPONSIBILITIES:

 

  • Be part of a team that works to establish a system and be responsible for the coordination of medical/social wellness appointment for residents who require transportation outside of their primary service area.
  • Integrate into, and work to assist with the establishment of effective team(s) dedicated to the safe, affordable, and reliable transportation of Rainy River district residents seeking medical/social services and care outside of their primary service area.
  • Generate a directory of regional health and social services regularly attended to by residents of the Rainy River district.
  • Establish connections with regional health/social wellness agency leaders, promote and install referral pathways.
  • Assist with the compiling, analyzing and research of data related to regional health/social services for the purpose of coordinated access to regional appointments.
  • Help to identify solutions and gaps in service for those seeking safe, affordable, accessible, and reliable transportation to regional, in-person health/social wellness engagements.
  • Produce a public information campaign aimed at district residents who require coordination and transportation solutions.
  • Attend to and provide necessary care to district residents while they travel outside of their primary service area for care at medical and social care providers.
  • Carry out all work assignments safely and be knowledgeable and/or in compliance with relevant regulations, policies, and procedures.

SPECIFIC DUTIES:

 

  • Coordinate transportation access for individuals in the Rainy River district who are required to travel outside of the district to attend health and social service appointments.
  • Coordinate and participate in the development of an online booking tools for access and scheduling of trips with the Rainy River District Ontario Health Team (RRDOHT) transportation program.
  • Assist with the formation of a directory of regional health and social wellness providers for promotion to district residents and care providers with context to regional travel. Include details for each one the providers, including:
    • Contact details for the appointment booking agent.
    • Modalities of care options.
    • Service location options for in-person consults/treatments.
    • Clinic/office hours of operation.
    • Any other relevant information specific to the provider, such as,
      • Accessibility Issues
      • Language services
      • Fee for service
  • Meet with local and regional health and social service providers to inform on goals and updates of the RRDOHT transportation initiative
  • Coordinate a public information campaign to promote the RRDOHT transportation initiative using print, radio, and social media platforms to publicize the program. Visit senior gathering centres and health and social wellness provider front-line staff for information exchange and encouragement of referrals.
  •  Integrate and share referral pathways with regional and local health/social services providers for access to coordinated transportation form the Rainy River district for medical and social wellness appointments, as well as repatriation of district residents.
  • Coordinate referrals for transportation services for residents of the Rainy River District.
  • Assist with the development and tracking of Key Performance Indicators using data collected from local and regional health and social service providers.
  • 'Coordinate travel grant and Non-Insured Health Benefit compensation and the application to the coordination and transportation of district residents. Collect and reconcile fees related to the RRDOHT transportation program.
  • Attend trips to regional health and social service providers to support and facilitate client needs and wellbeing while enroute to coordinated appointments outside of the primary service area of district residents. Such supports may include,
    • Assisting with medication compliance
    • Coordination and distribution of nutritionally appropriate meals and snacks.
    • Assist in the proper utilization and care of adaptive aid equipment required by the client.
    • Report any changes in the clients’ overall health to the caregiver (as defined on the referral/intake form)
    • As required, provide personal care to the client by assisting with activities of daily living including eating, toileting, and ambulation.
  • Participate in departmental and related committees/meeting when required.
  • Actively participates in risk management and continuous quality improvement processes and activities.
  • Participate in educational activities and programs. Attend and participate in appropriate courses, conferences, and workshops to promote professional development.
  • Report any unusual incidents or instants of risk/harm to Supervisor or designate, and document using the Adverse Events Management System.
  • Carry out all work assignments safely and be knowledgeable of an in compliance with relevant regulations, policies, and procedures. (i.e. Occupational Health and Safety Act.)
  • Other such duties or responsibilities as may be delegated from time to time by the Manager of Community Support Services or designated.

POSITION AVAILABLE:

This Full-Time Interim position will commence: As soon as possible and will terminate March 28, 2025, or with two (2) weeks written notice by the employer
