Full-Time Pharmacist

DEPARTMENT/SERVICE AREA:

REPORTS TO (Title):

Manager, Therapeutic Services

The Pharmacy department has an opening for a Full-Time Pharmacist. The successful candidate must be able to meet the normal requirements of the job and possess the following qualifications:

QUALIFICATIONS NECESSARY:

  • Bachelor of Science Degree - Pharmacy;
  • Licensed in Ontario;
  • Minimum 3 years hospital Pharmacy experience an asset;
  • Effective leadership and interpersonal skills;
  • High professional ethics and standards of practice;
  • Excellent communication skills;
  • Excellent record of attendance
  • Working knowledge and understanding of legislation as it applies to hospital and healthcare settings;
  • Dependable, competent and continued ability to carry out responsibilities;
  • Physically capable of performing the essential duties of the job.

BASIC RESPONSIBILITIES:

1. To supervise and coordinate day to day activities of the Pharmacy Department.

2. To provide clinical pharmacy services to La Verendrye, Emo and Rainy River sites.

SPECIFIC DUTIES:

Human Resources/Leadership

1. Assume responsibility for the day-to-day operations of the department in the absence of the Manager, Pharmacy.

2. Provide training and support for new staff members, students, etc. when required.

3. Provide updated drug information to other Health Care professionals via in services or informal meetings.

Care Management

1. Assess physician's medication orders with respect to appropriateness.

2. Perform or supervise the preparation and dispensing of medications and verify completed orders for correctness before issuing.

3. Ensure proper functioning of the drug distribution system including procurement, storage, control and disposal of drugs throughout the three health centres.

4. Perform or supervise the inspection of pharmaceuticals stored in any area of the health centres as required. (eg. Narcotics, night cupboard, etc.)

5. Investigate and report on adverse drug reactions and medication errors.

6. Prepare written drug/disease information material for newsletters.

7. Provide information regarding compatibility and stability of IV admixtures as required.

8. Prepare chemotherapy administered in the Hospital in a ready to use format.

9. Coordinate or prepare any unique I.V. compounds used within the Hospital.

10. Coordinate provision of I.V. additive services, including TPN.

11. Monitor drug therapy and provide recommendations as required.

12. Provide medication counselling to in-patients and out-patients as required.

13. Complete medication histories on in-patients as required.

14. Provide medication histories, medication information and counselling to chemotherapy patients as required.

15. Provide updated drug information to other Health Care professionals via in services or informal meetings.

Fiscal/Resource Management

1. Evaluates, selects, purchases and stores appropriate pharmaceuticals and ensure appropriate, secure storage.

2. Maintains an adequate stock of medications, antidotes and emergency drugs in a cost effective manner.

Quality/Risk Management

1. Maintain knowledge of current organizational and pharmacy policies and procedures and ensure compliance; recommend improvements.

2. Assist in the development, implementation and evaluation of pharmacy programs and services for the three health centres.

3. Participate in departmental Quality Improvement Initiatives supportive of strategic goals. Drug utilization review, workload measurement and undertake special projects, as assigned.

4. Ensure the safe handling of hazardous and toxic pharmaceutical in accordance with legislative requirements.

5. Participate in Accreditation activities and ongoing Quality Assurance.

6. Accountable to integrate the principles of patient safety and risk management into daily work roles and responsibilities.

Organizational and Professional Development

1. Maintain professional competence by keeping informed of changes in medical practice, new drugs and their role in current therapy.

2. Maintain professional competency by attending conferences, seminars and by keeping updated on current drug information.

Other Duties:

1. Perform other duties as may be from time to time designated by the Manager, Pharmacy.

CURRENT ASSIGNMENT:

§ This Full-Time position may be required to work Monday – Friday (0730 – 1530).

POSITION AVAILABLE:
