REPORTS TO (Title):

Resident Experience & Activation Coordinator

The Activation Department has an opening for a Part-Time BSO/Activation Worker. The successful candidate must be able to meet the normal requirements of the job and possess the following qualifications:

QUALIFICATIONS NECESSARY:

  •  Post-secondary diploma or degree in recreation and leisure studies, therapeutic recreation, kinesiology or other related field from a community college or university or enrolled in such a field, preferred.
  • Demonstrated extensive knowledge working with older adults with various forms of dementia, delirium, complex mental health, substance use, other neurological conditions, including those with or at risk of responsive behaviours;
  • Specialized training in Gentle Persuasive approaches (GPA), U-FIRST, PIECES, Montessori, DementiAbility are an asset.
  • Dementia, seniors care, positive approach to care related courses or certifications an asset or willingness to receive additional training;
  • Experience in Activation programming as it relates to programs offered in long term care settings
  • Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for residents and families.
  • Relevant clinical experience (3-5 years) working with older adults and care partners in long-term care and community settings;
  • Knowledge of the Principles of Adult Learning and experience educating adults.
  • Knowledge and understanding of pertinent legislation and policies, including the Fixing Long-term care (FLTC) Act and regulations, the Personal Health Information and Protection Act (PHIPA, 2004), the Mental Health Act and other pertinent legislation
  • Excellent organizational and time management skills;
  • Proficient computer skills;
  • Experience with electronic documentation, specifically the Resident Assessment Instrument (RAI) documentation, including assessments, Resident Assessment Protocols (RAPS), activity and care planning functions;
  • Demonstrated strong written and verbal communication skills;
  • Demonstrated ability to maintain strict confidentiality;
  • Demonstrated ability to function effectively independently, within a team, with front line personnel, and management;
  • Dependable with a verifiable acceptable attendance record;
  • Proven ability to perform the essential duties of the position on a regular basis;

BASIC RESPONSIBILITIES:

1   . Provides recreational services, which may include: individual/group treatment, diversional activity, social interaction, therapeutic adaptations, therapeutic skill development, leisure education and community access/linkage and referral

2   . Advocate for and, protect the individual rights of residents.

3   . Participates as a member of the interdisciplinary health team, fulfilling responsibilities as a member of the nursing discipline.

4   . Carry out all work assignments safely and be knowledgeable and/or in compliance with relevant regulations, policies, and procedures.

5   . Exercise behaviour consistent with Riverside Health Care’s Code of Conduct.

DUTIES AND RESPONSIBILITIES:

  1.    . Incorporates physical, cognitive, affective, social, and spiritual domains into individual/group sessions to promote a balanced/optimal leisure lifestyle
  2.    Therapeutic activities are based on interest, need, and ability in cooperation with the interdisciplinary team and volunteers.
  3.   . Enhance the positive impact and spread of Behavioural Supports Ontario (BSO) strategies for one on one and group therapeutic modalities to address and prevent responsive behaviours, while adapting to individual changing needs
  4.   . Formulate, implement, monitor and evaluate individualized therapeutic program plans and goals in conjunction with the multidisciplinary team
  5.    . Actively participate in resident care conferences and referral based complex case reviews
  6.    . Maintain accurate, complete, organized clinical records/documentation relating to assessment and interventions.
  7.    Identify and coordinate the need for additional consultation and/or referrals to external services or specialties and coordinate referrals.
  8.   . Monitor, document and evaluate therapeutic activities.
  9.   . Participate in the development and delivery of educational and informational programs for residents, families, volunteers and staff
  10.   . Maintain effective relationships with internal partners, community partners, volunteers and families.
  11.    Incorporates current research findings into clinical practice.
  12. .   Support the spread of improvement practices with internal team members regarding overall prevention of responsive behaviours.
  13. . Facilitate transfer of knowledge into practice and enhance staff’s ability to manage responsive/expressive behaviours and care planning interventions through role modelling, coaching, and mentorship.
  14. . Provides education and coaching to LTC home staff in the use of frameworks such as PIECES and UFIRST to assess responsive behaviours.
  15. . Actively participate in all relevant professional development as coordinated at Riverside Health Care as well as regional, hub and local levels as guided by NW BSO Structure
  16. . Identify risks and gaps within the home related to responsive behaviours and the education and training required to mitigate risk, create capacity, and build support.
  17. . Maintains client care/non-client care statistics.
  18. . Demonstrated awareness and competencies regarding cultural diversity, including francophone, LGBTQ, and Indigenous-specific considerations
  19. . Demonstrated resident-family-care partner focus and commitment to the Mission, Vision, Values, and goals of Riverside Health Care, BSO and respective partner agencies
  20. . Actively participates in risk management and continuous quality improvement processes and activities
  21. . Responsible for following all Occupational Health and Safety regulations and relevant organizational policies and procedures to ensure personal safety and safety of others
  22. . Other such duties or responsibilities as may be delegated from time to time by the Supervisor or designate.
