REPORTS TO (Title):

Manager, Mental Health and Addictions

The Community Support Services Department is actively recruiting a Part-Time Rapid Access to Addiction Medication (RAAM) Nurse Practitioner. The successful candidate must be able to meet the normal requirements of the job and possess the following:

QUALIFICATIONS NECESSARY:

 

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice (required)
  • Clinical experience is considered an asset
  • Demonstrated knowledge and experience with opioid and opioid-related addictions, issues and challenges
  • Demonstrated knowledge of community-based resources and the regional health care system
  • Experience providing care with, and knowledge of Indigenous culture is considered an asset
  • Ability to work closely with our Anishinabee partners and to ensure planning is flexible, sensitive and allows for innovation of appropriate cultural diversity and protocols, and implementation is culturally relevant and appropriate
  • Willingness to actively participate in best practice, including learning opportunities that foster Anishinaabe best practice methodologies while respecting the diversity, cultural norms, cultural protocols, and cultural values of each individual, family, and community service
  • Anishinaabemowin – Ability to communicate effectively in Ojibway is an asset
  • Trauma informed and ability to respond to crisis situations
  • Demonstrated knowledge of Informed Decision Making, Patient Capacity, Patient Rights and Consent for Treatment
  • Maintains CPR certification designed for health-care professionals and standard first-aid
  • Experience with, or knowledge of dual diagnosis & concurrent disorders is considered an asset
  • Demonstrated strong interpersonal skill with the ability to function effectively independently, within a team, with front line personnel , and management
  • Experience working independently and with minimal supervision
  • Proven ability to develop and maintain effective working relationships with others both internal and external to the organization
  • Knowledge of, and in compliance with relevant legislation in health and safety and employment, (OH&S Act and its Regulations, WSIB, Human Rights) 
  • A Satisfactory Vulnerable Sector Screening (Police Check) report
  • Valid Ontario driver’s license
  • Physically capable of performing essential duties of the job

 

BASIC RESPONSIBILITIES:

 

  • Attends to clients with addictions and concurrent disorders at the RAAM clinic, with or without scheduled and urgent appointments.
  • Attends to clients with addictions and concurrent disorders in the community during regular and extended hours.
  •  Works within the scope of practice of the RN (EC), performs a comprehensive health or symptom directed assessment of the client and synthesizes data from multiple sources to make diagnosis.
  • Ensures that standards of patient care are maintained according to the Standards of Nursing Practice, the Standards of Practice for Nurses in the Extended Class, as well as the Regulated Health Professionals Act and the Nurses' Act.
  • Assist clients/patients to engage pro-actively with community, family and friends through care mapping and coaching
  • Determines the need for, and orders appropriate diagnostic tests and procedures in accordance with an approved list or in consultation with a physician where necessary; interprets the results of screening and diagnostic laboratory tests, within the scope of practice.
  • Participate on agency and community committees and working groups as requested by Manager
  • Maintain accurate and timely records. Write reports, support letters, and chart summaries for transfer and/or legal purposes, as well as other related patient and workload measurement documents, as required.
  • Maintains confidentiality and acts in an ethical manner consistent with professional expectations

 

SPECIFIC DUTIES

 

  1. Spends adequate time with the client to determine the presence of existing and potential health problems, with a major focus on substances used, related lifestyle, psychological, sociocultural, and environmental risk factors which may influence the client’s health status, deals with these personally or by referral to or in consultation with, other health care providers and agencies/services
  2. Prescribes drugs included in the Schedules of the Regulations to treat diseases or disorders within the RN(EC) scope of practice.
  3. Provides advanced nursing care and treatment (including pharmacological complementary therapies and/or counselling interventions for health problem).
  4. Monitors the ongoing therapy of clients with addictions, chronic mental and physical illness by providing effective pharmacological, complementary, or counselling interventions.
  5. Demonstrate an understanding of opioid and other related drug addictions, complex care planning and management.
  6. Ensure that all patients/clients have equal and fair access to health care and essential social services and community resources.
  7. Build capacity among primary care to transition clients for ongoing monitoring and treatment
  8. Contribute to patient/client data collection for quality and reporting requirements
  9. Travel in the region, as required
  10. Other duties as assigned by Manger of Mental Health and Addictions or designate

 

REPORTING:

This position reports to the Manager, Mental Health, and Addictions.

POSITION AVAILABLE:

This Part-Time position will commence: As soon as possible.
